How the treatment runs
Each stage below is a knot on the thread. Nothing is skipped, and you are told at the outset roughly how long yours will take.
Stage 1
Examination and mapping
Both openings are identified and the tract is probed. Where the tract is high, branching, or recurrent after previous surgery, an MRI fistulogram is arranged before anything is placed.
Stage 2
Thread placement
Under local or spinal anaesthesia, the medicated thread is passed through the tract and tied into a loop. This is a day-care procedure — you go home the same day.
Stage 3
Weekly thread change
Each week the thread is replaced with a freshly coated one. The visit takes a few minutes. The tract is cut a little further while the portion already cut heals shut behind it.
Stage 4
Tract closure
The thread completes its passage through the tract and falls away, leaving healed tissue rather than an open wound. Timing depends on tract length and complexity.
Stage 5
Confirmation follow-up
Reviews continue for several weeks after closure to confirm no residual branch has been left behind — the step most often skipped elsewhere, and the most common reason a fistula returns.
Why does the thread cut and heal at the same time?
The alkali on the thread debrides tissue at the cutting edge while the chemical action behind it promotes granulation. Because cutting advances at a fraction of a millimetre per day, healing tissue always keeps pace with it, so no open gap forms across the divided muscle.
This is the mechanical difference from a scalpel. A scalpel divides tissue instantly, leaving two separated ends that must then heal across a gap. If those ends are sphincter muscle, the gap is what threatens continence.
The thread never creates that gap. At every point in the weeks of treatment, the tissue behind the cutting edge has already knitted. That is why the method is preferred for high tracts that pass through significant muscle.
How long does Kshara Sutra take?
Treatment duration depends on tract length. A simple low tract may complete in six to eight weeks. A long, high or branching tract, or one recurring after previous surgery, can take several months. The thread advances at a predictable rate, so an estimate is given after mapping.
Kshara Sutra is not reserved for complicated fistulas. It treats simple low tracts as readily as high or branching ones, and the trial with the strongest published results for it was carried out in simple cases. What complexity changes is how long the course runs, not whether the method applies.
For a short, simple, previously unoperated tract, that trade may not be worth it, and you will be told so. The purpose of the first consultation is to establish which situation you are actually in.
Common questions
Mild discomfort and a dragging sensation are normal, particularly in the first two weeks. Sharp pain is not expected and should be reported. Pain relief and sitz baths are prescribed from the outset.
Short travel is usually fine between changes, but the weekly appointment cannot be skipped without slowing progress. If you travel frequently, discuss it at the consultation so the schedule can be planned around it.
