Why does a fissure keep coming back?
A fissure creates a cycle: the tear causes pain, pain causes the internal sphincter to spasm, spasm reduces blood flow, and poor blood flow stops the tear healing. Breaking that cycle — not just dressing the tear — is what makes treatment last.
This is why simple ointments often give a few days of relief and then fail. They soften the stool or numb the pain but leave the sphincter spasm and the poor blood supply untouched, so the tear reopens with the next hard stool.

When does a fissure need a procedure?
A fissure that has lasted more than six weeks, or one that has a sentinel tag and hardened edges, is considered chronic and rarely heals on medication alone. At that stage Kshara Karma is applied to the fissure bed to create a fresh, cleanly healing wound.
The application is deliberately mild. It creates a controlled fresh surface without destroying surrounding tissue, so healing runs from a clean base rather than across scarred, hardened edges.
Where the sphincter spasm is severe, gentle graded anal dilatation is added during follow-up. Cutting the sphincter — the conventional sphincterotomy — is avoided wherever possible, because that is the step most associated with long-term continence problems.

A fresh fissure treated early often settles within two to three weeks with diet correction and medication. A chronic fissure treated with Kshara Karma usually heals over three to four weeks, with pain easing well before the tear closes fully.
