Kshara Sutra Treatment for Anal Fistula in Jayanagar, Bengaluru
An anal fistula is a small tunnel connecting an infected anal gland to the skin near the anus, called Bhagandara in Ayurveda. It discharges pus or blood repeatedly and does not heal on medication. Symptoms

Why does fistula come back after surgery?
Conventional fistula surgery cuts out the tract in one operation. If any branch of the tract is missed, or if the tract runs through sphincter muscle that the surgeon must spare, the fistula recurs. Reported recurrence after conventional surgery runs as high as thirty per cent.
The surgeon faces a genuine dilemma. Cut wide enough to remove every branch and you risk damaging the sphincter and the patient's continence. Cut conservatively to protect the sphincter and you risk leaving tract behind, which reopens weeks or months later.
Kshara Sutra sidesteps the dilemma rather than solving it by force. Because the thread cuts a fraction of a millimetre per day and healing follows immediately behind it, the muscle is divided and repaired continuously rather than severed at once. There is no open gap for the ends to separate across.
What does Kshara Sutra treatment actually involve?
A thread coated in medicated alkali is passed through the fistula tract and tied. It cuts through the tract slowly while the tissue behind it heals. The thread is replaced weekly in a short outpatient visit until the tract has been cut through completely and healed shut.
The first visit involves examining the tract, identifying both openings and, where the anatomy is complex, arranging an MRI fistulogram before anything is done. The thread is then placed under anaesthesia as a day-care procedure.
After that, treatment is a weekly outpatient appointment lasting a few minutes. You continue working. The total time depends on the length and complexity of the tract — a simple low tract may finish in six weeks, a long or branching one takes longer.
Is Kshara Sutra better than laser fistula surgery?
Kshara Sutra treats the full range of fistula, simple and complex alike, dividing the tract gradually so the sphincter is never cut across in one action. Laser is faster and finishes in one sitting, though pooled reviews report healing near 63 to 70 per cent with about a third of patients needing a further procedure. No head-to-head trial has compared the two.
Neither method is universally better, and anyone who tells you otherwise is selling. The honest answer depends on your tract, and that cannot be determined without examination.

Yes. After the thread is placed, treatment is a weekly outpatient visit of a few minutes. Most patients continue working throughout. You may be advised to avoid long periods of sitting and heavy lifting in the first week.
Kshara Sutra is specifically designed to avoid that risk. Because the thread divides tissue gradually while healing follows behind it, the sphincter is not severed in one cut. This is the principal reason the method is chosen for high and complex tracts.
At Swasthi Ayurveda the Kshara Sutra procedure is ₹35,000 to ₹60,000 depending on tract complexity, with each weekly thread change charged separately from ₹1,500. Across a six to twelve week course the full treatment typically totals ₹44,000 to ₹78,000. Consultation is ₹700 and includes a written estimate covering the whole course.
Recurrent fistula after failed surgery is one of the main reasons patients come to us. Previous surgery makes the anatomy harder to map, so imaging is usually arranged first, but a recurrent tract is not a reason to rule out Kshara Sutra.
