Questions patients actually ask
These are the questions that come up in clinic, answered the way they are answered in the room. Where the honest answer is “it depends on examination”, that is what it says.
Common questions

Common questions
Kshara Sutra is a medicated thread passed through a fistula or sinus tract, changed weekly, which cuts and heals the tract simultaneously. Kshara Karma is a medicated alkaline paste applied to a pile mass or fissure bed in a single day-care sitting. Different problems, different methods.
No. Swasthi Ayurveda is a day-care facility. Procedures are performed under local or spinal anaesthesia and you go home the same day.
Procedures are performed under anaesthesia, so you should not feel pain during them. Discomfort afterwards is usual and is managed with medication and sitz baths. Most patients report considerably less pain than they expected, and less than after conventional excision surgery.
After Kshara Karma most patients return to desk work within one to two days. During Kshara Sutra treatment most patients work throughout, attending a short weekly appointment for the thread change.
Early first-degree piles and fresh fissures often settle with diet correction and oral medication. Established prolapsing piles, chronic fissures and fistula tracts do not close on medication alone and need a procedure. You will be told honestly which category you are in at the first visit.
Yes. The clinic also provides Panchakarma, Agnikarma and Viddhagnikarma for musculoskeletal pain, leech therapy and Raktamokshana, varicose vein management, diabetic and non-healing wound care, and Swarna Bindu Prashana for children.
Common questions
Recurrence is possible with any method, including surgery. It is least likely when the underlying cause is corrected, which is why every plan includes diet and bowel-habit guidance. No clinic can honestly guarantee a permanent cure, and you should be cautious of any that does.
The method exists specifically to reduce that risk. Because the thread divides tissue gradually while healing follows immediately behind it, the sphincter is never severed in a single cut. This is why it is preferred for high and complex tracts.
If examination suggests something outside our scope — suspected malignancy, inflammatory bowel disease needing gastroenterology, advanced vascular disease, or a complication requiring a hospital — you will be referred. Treating something we should not is a worse outcome than losing a patient.
Only for non-intimate wound sites such as diabetic and varicose ulcers, and only with written patient consent. Photographs of ano-rectal conditions are never published on this website. They can be shown during your consultation if you wish to see them.
