Why the practice is narrow

Most Ayurvedic clinics in Bengaluru offer a long menu — weight management, hair care, skin, fertility, detox packages. There is nothing wrong with that, but it is a different kind of business from this one.

Ano-rectal work is unglamorous, technically demanding, and depends on examination skill that only accumulates by doing a great deal of it. Fistula in particular punishes the generalist: tracts branch, recur, and are missed. So the practice was built around it rather than around it being one option among forty.

Sushruta's methods, used as intended

Kshara Sutra and Kshara Karma are described in the Sushruta Samhita, the surgical text of Ayurveda, and they are not folk remedies retrofitted with modern language. They are procedures, performed under anaesthesia, with defined indications and defined limits.

They are also not universal. A short, simple, previously unoperated fistula may be better served by a laser procedure elsewhere. Saying so, rather than treating everyone who walks in, is the practice's actual position on tradition: use it where it is genuinely better.
