Kshara Karma is the controlled application of a medicated alkaline paste directly onto a pile mass or fissure bed. The alkali cauterises the tissue chemically so it shrinks and separates, without any incision or stitches. It is a day-care procedure performed under local anaesthesia.

How the treatment runs
Each stage below is a knot on the thread. Nothing is skipped, and you are told at the outset roughly how long yours will take.
Stage 1
Grading and consent
The pile mass or fissure is examined and graded, and the number of likely sittings and the full cost are given to you before anything begins.
Stage 2
Application under local anaesthesia
The Kshara paste is applied for a carefully timed period, then neutralised. Timing is what separates a controlled application from a burn, and it is not rushed.
Stage 3
Same-day discharge
You rest briefly and go home. A burning sensation for a few hours is expected and is managed with medication and sitz baths.
Stage 4
One-week review
The treated area is inspected as the slough separates and the base begins to heal. Diet is reviewed and adjusted.
Stage 5
One-month review
Healing is confirmed and the long-term bowel and diet plan is set, which is the part that determines whether the problem stays away.
How is Kshara Karma different from surgical removal?
Surgical haemorrhoidectomy cuts the pile mass out, leaving an open wound in a sensitive area that is painful for one to three weeks. Kshara Karma chemically shrinks the same tissue in place, so there is no surgical wound, no stitches, and markedly less post-procedure pain.
The trade-off is that Kshara Karma may need more than one sitting for higher grades, whereas surgery addresses everything in a single operation. For most patients the reduced pain and faster return to work outweigh the extra visits.
Because no sphincter tissue is divided, the continence risk associated with aggressive anal surgery does not arise in the same way.

Most patients need one to three, depending on the grade and the number of pile masses. The likely number is given at your consultation, not discovered as you go.
Piles in pregnancy are usually managed conservatively with diet, medication and positioning, with procedures deferred until after delivery unless there is a compelling reason. Dr. Sumukhi will advise based on your stage of pregnancy.
