How do I know if it is piles and not something more serious?
Painless bright red bleeding with a soft lump usually points to piles. But bleeding from the rectum can also come from a fissure, a fistula, or something that needs ruling out urgently. Any rectal bleeding should be examined in person — never diagnosed from a web page or a photograph.
Piles are typically painless. Pain usually means something else is present as well — most often a fissure, or a pile in which a clot has formed. That distinction changes the treatment entirely, which is why the first visit always includes a proper proctological examination rather than a prescription over the phone.
If you are over 40, if the bleeding is dark rather than bright red, if you have lost weight without trying, or if there is a family history of bowel cancer, Dr. Sumukhi will refer you for further investigation before starting any Ayurvedic procedure. Treating piles is not worth missing something else.
What is Kshara Karma and why use it for piles?
Kshara Karma is the controlled application of a medicated alkaline paste, prepared mainly from Apamarga, directly onto the pile mass. The alkali chemically cauterises the tissue so it shrinks and falls away. Nothing is cut, so there is no open surgical wound and no stitches to heal.
The method is described in the Sushruta Samhita, where Acharya Sushruta lists Kshara among the four ways of managing Arsha alongside medicine, cautery and excision. He rated Kshara highest, because it works where medicine alone cannot but avoids the blood loss and tissue damage of cutting.
In practice this means a short day-care procedure under local anaesthesia. Most patients walk out the same day and return to desk work within a day or two. Because no sphincter muscle is cut, the risk of the continence problems that follow aggressive surgery is very low.
Can piles be cured without any procedure?
Early first-degree piles often settle with diet correction, oral Ayurvedic medicine and better bowel habits alone. Second-degree onwards usually need Kshara Karma. No honest practitioner can promise that medicine alone will cure an established, prolapsing pile mass.
The first consultation always begins with the least invasive option that can realistically work. If your piles are early and the main driver is constipation, you will be given a pathya (diet) plan, oral medication and a follow-up date — not a procedure quote.
What we will not do is keep you on medication for months while the condition advances. If the grade calls for Kshara Karma, you will be told so at the first visit, with the cost, the number of sittings and the recovery time given upfront.
