What is fistula-in-ano?

A fistula-in-ano is an abnormal tunnel that forms between the anal canal and the skin near the anus, usually following an anal abscess that has drained but not healed. In Ayurveda it is described as Bhagandara, and it is documented in the Sushruta Samhita along with the Kshara Sutra technique still used to treat it today.

Unlike a fissure or piles, a fistula will not resolve on its own. The tract remains as a channel through which pus and discharge continue to pass, and it typically recurs until the tract itself is treated.

Kshara Sutra is a medicated thread — a surgical seton coated in successive layers of alkaline herbal preparations, traditionally including Apamarga kshara, Snuhi latex and Haridra. The thread is threaded through the fistula tract and tied. It gradually cuts through the tract while the tissue behind it heals simultaneously, so the tract is eliminated progressively rather than excised in one operation.

The procedure, step by step

• Assessment. Examination and, where required, imaging to map the tract, identify the internal opening and determine whether the fistula is simple or complex.

• Thread placement. Under local anaesthesia, the Kshara Sutra is passed through the tract and tied. This is a day-care procedure — you go home the same day.

• Weekly thread changes. You return once a week for the thread to be replaced. Each fresh thread continues the cutting and healing action.
